Humana's Primary Care Organization is one of the largest and fastest-growing senior-focused, value-based care providers in the country.
We are looking for a primary care physician to join our team full-time at Conviva/CenterWell Primary Care. This role is ideal for a compassionate, experienced clinician who excels in a collaborative, patient-centered environment and is committed to improving outcomes for adult and geriatric populations. You will report to the area medical director.

Responsibilities
* You will deliver comprehensive outpatient care to adult and senior patients.
* You will maintain accurate medical records and documentation.
* You will diagnose and manage moderately to complex medical conditions.
* You will oordinate referrals and collaborate with specialist.
* You will participate in clinical quality improvement projects.
* You will work with interdisciplinary teams to ensure holistic care.
* You will contribute to strategic programs and innovations in care delivery.
Use your skills to make an impact
Required Qualifications
* MD or DO from an accredited medical school.
* Active, unrestricted medical license in the state of practice.
* Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
* Two years' experience in care or managing high-acuity geriatric patients.
* Commitment to improving patient experience and outcomes.
* Participation in Tuberculosis (TB) screening program.
This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
You must be passionate about contributing to an organization focused on improving consumer experiences
You must be willing to participate in limited on-call coverage by phone
Other qualifications:
* Specialty training in family medicine, internal medicine, med-peds, or geriatrics.
* Experience working with senior populations or in value-based care settings.
* Proficiency with electronic health records (EHR) and digital documentation.
* Adapt to evolving clinical environments.
* Experience supervising advanced practice providers

Work Environment
* Outpatient clinical setting.
* Standard schedule: Monday-Friday, 8:00 AM - 5:00 PM.

Pay RangeThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$255,000 - $298,500 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
