The Care Coach 1 assesses and evaluates member's needs to achieve and maintain wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 work assignments are often straightforward and of moderate complexity.
The Telephonic Care Coach 1 employs a variety of strategies and techniques to support a member's wellness state by coordinating services and resources. Identify and resolve barriers that hinder care. Ensure patient is progressing towards desired outcomes by overseeing patient care through use of assessment, data, conversations with member, and active care planning. Understand own work area professional concepts/standards, regulations, strategies and operating standards. Makes decisions regarding own work approach/priorities and follow direction. Managers manage and often guide work according to precedent and documented procedures/regulations/professional standards with some interpretation.

Required Qualifications
* Must reside in the state of Kentucky or within a county contiguous to the Kentucky state border in Ohio, Indiana, Illinois, Missouri, Tennessee, Virginia, or West Virginia, within a 15-mile radius
* 2+ years of related experience in a Care Coach or similar role
* Proficiency in Microsoft Office, including Word and Excel
* Ability to use multiple electronic software applications to perform job responsibilities
Preferred Qualifications
* Bachelor's degree in social sciences, social work, human services, or a related field
* Experience working with Medicare and Medicaid recipients
* Experience with electronic case documentation and documenting across multiple systems
* Experience in health promotion, coaching, and wellness
* Knowledge of community health, social service agencies, and local resources
* Bilingual proficiency. Must be able to speak, read, and write fluently in English and one additional language. The hiring team will require selected candidates to complete a language proficiency assessment in both languages.
Additional Information
* Schedule/Time Zone: Monday through Friday, 8:00 AM - 5:00 PM Easter Time Zone with flexibility to work overtime as per business needs.
* Work Location: Kentucky
* Work Style: Remote
* Travel Requirements: 5% travel includes attending face-to-face and collaborative meetings at Humana's Louisville, KY location.

Pay RangeThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$53,700 - $72,600 per year
