Job Summary and Qualifications

The Intake Assessment Evaluator in conjunction with the Program Director and Intake Assessment Coordinator coordinates the pre-admission assessment process and provides clinical assessments for potential patients. Also, assists with community education with clinical issues and participates with community events such as health fairs. The Intake Evaluator responds to all calls for information, referral of patients, and inquiries about services offered and assists callers with accessing the most appropriate level of service and service provider to meet the needs of the patient.

* The intake assessment evaluator will respond to all calls for psychiatry services responding appropriately to the identified needs of the caller and patient. Patient calls will be prioritized based on the level of need of the patient. Insures EMTALA compliance at all times and reports any concerns to the program director immediately.
* Coordinates communication related to all assessment and admission activities, insures rapid throughput for the emergency department, requests additional resources when needed based on volume, and works with the medical staff to move patients to disposition quickly.
* Responds to calls for assessments in the emergency department within 15 minutes and initiates the requested assessment as soon as possible and no later than 60 minutes from the initial request. Implements a back up procedure when the 60 minute time frame cannot be met due to heavy volume.
* Works with inpatient nursing staff to insure an appropriate bed is available for patients coming in including moving patients as needed to make beds available quickly.
* Intake coordinator is responsible to document all calls coming in regardless the outcome in the approved format. Any staff member providing intake services will document all calls according to the system the intake coordinator has in place.
* Intake coordinator is responsible to provide education and support related to emergency detention procedures, intervention as needed, completion of paperwork, and addressing process concerns with the emergency department director and medical staff to insure compliance with all regulatory requirements.
* Works collaboratively with behavioral health sales team to provide data, assure accurate referral data management, and includes sales team members in team meetings as needed.
* Completes assessments in a timely manner - 90 minutes or less on average
* Chart review 15-20 minutes
* Interview 20-30 minutes
* Documentation 20-30 minutes
* Demonstrates ability to set priorities.

* Follows established policy and procedures in performing job functions.

* Ensures patients rights and confidentiality.
* Able to assess and reassess needs of patients.
* Provides on and off site clinical assessments for potential patients to the program.
* Works collaboratively with hospital departments to assist managing behavioral health assessment needs.
* Participates as an active member of the multi-disciplinary team.
* Provides ongoing education to patients and families as needed.
* Provides resources and support for patients before, during and after treatment.
* Coordinates and participates with appropriate interested parties and physicians through the patient referral inquiry, admission, treatment and discharge process.
* Models and trains other behavioral health team members on managing and tracking inquiry calls.
* Tracks and reports on all key indicators related to calls, assessment, admissions.
* Performs other duties as assigned by Program Director.ADOLESCENT

* Facilitates visitation of friends and family.
* Observes for cues of substance abuse, depression and eating disorders.
* Provides accurate information, reassurance and support when there is anxiety regarding potential altered body image. Encourages questions.
* Recognizes the patient/s needs to assert independence, reluctance to express dependence needs and anxiety about hospitalization.ADULT
* Recognizes potential for increased stress related to dependency needs of others; i.e., children and elderly parents.
* Assesses altered perceptions resulting from change from independence and productive status to dependence and curtailed productivity.
* Recognizes anxiety regarding potential changes in body image and lifestyle resulting from illness.GERIATRIC
* Identifies increased physical/emotional dependency needs and availability of support networks.
* Assesses potential for compromised skin integrity and initiates preventative measures when needed.
* Determines increased risks for nutritional deficiencies.
* Notes changes in elimination patterns.
* Identifies increased sensitivity to climate changes.
* Determines presence of disabilities affecting mobility or sensory acuity.ALL AGE GROUPS
* Assesses potential for increased educational needs due to compromised ability to comply with therapy and interventions.
* Completes admission assessment and reassessments within designated time frames per policy.
* Responds promptly and accurately to meet patient's needs.
* Demonstrates ability to modify interview technique to assess/identify physical, psychological and educational needs.
* Provides for safety measures.
* Ability to involve patient/family/significant other in decision making related to patient care.PROFESSIONAL RESPONSIBILITIES
* Cooperates with other health team members in achieving common goal of quality health care.
* Assumes responsibilities for quality care.
* Adheres to legal responsibilities as written in The State of Florida Nurse Practice Act as applicable to position.
* Demonstrates clinical knowledge of behavioral health services and maintains the highest quality of comprehensive patient care.
* Utilizes knowledge of growth and development, psychopathology, and psychosocial systems.
* Has an accurate knowledge of psycho-dynamics and therapeutic processes as demonstrated by effective intervention and resolution of problems as they occur, and communication of the processes to others.
* Has a comprehensive knowledge of existing policies and procedures and utilizes the standards stated as evidenced by independent functioning and carrying out the philosophy of the Behavioral Health Services Department.
* Demonstrates behavior that is consistent with leadership.
* Demonstrates good organizational skills based on the needs of the patients and program as evidenced by following through on assigned tasks in a smooth and effective manner and determining priorities.
* Demonstrates a professional attitude and emotional stability when coping with stress, accepting guidance and evaluating own performance.
* Acts as a role model, by utilizing professional standards, motivating peers and co-workers, stimulating growth and development in others.
* Responsible for maintaining upgraded knowledge in behavioral health by engaging in activities that will enhance current knowledge base.
* Attends unit meetings as specified by Program Director in order to receive current information.
* Complete all competency requirements in a timely manner.
* Helps promote Behavioral Health Program through referral development and community education events as assigned by Program Director.PATIENT/FAMILY EDUCATION:
* Provides patient/family education as per department and hospital policy and procedure.
* Assesses patient/family/significant other's learning needs, abilities and readiness to learn. Identifies barriers to learning, such as language or culture. Integrates this information into the patient's intake assessment.
* Identifies and documents patient/family/SO's education needs in collaboration with other disciplines and incorporates into the intake assessment.
* Documents patient/family education and provides educational materials as appropriate.
* Collaborates with other health care providers in education patients and families to behavioral health services.

What qualifications you will need:

* Basic Cardiac Life Support must be obtained within 30 days of employment start date
* Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
