CASE MANAGER - Ambito Sanitario Assistenziale

Beschreibung:

La posizione prevede di sviluppare e/o creare il percorso individuale di cura del/la cliente assicurativo/a, creando un processo assistenziale tarato sul singolo caso che si occupi dell'aspetto clinico, manageriale e finanziario.

RESPONSABILITA' PRINCIPALI
* Gestione dei casi sanitari assegnati, garantendo un supporto completo ai clienti con un servizio efficiente e di qualità
* Valutazione delle necessità cliniche e pianificazione delle cure in collaborazione con i fornitori di servizi sanitari
* Coordinamento e gestione delle comunicazioni tra clienti, medici e personale dedicato alla gestione della pratica
* Monitoraggio dei progressi clinici e aggiornamento degli stakeholder coinvolti
* Identificazione e implementazione di miglioramenti nei processi di gestione dei casi per ottimizzare l'efficienza operativa

REQUISITI CHIAVE / COMPETENZE / ESPERIENZA
* Laurea in ambito sanitario (ad es. Scienze infermieristiche)
* Esperienza consolidata nella gestione ed erogazione dei servizi infermieristici a domicilio o in struttura
* Eccellenti capacità di comunicazione e gestione delle relazioni
* Capacità organizzative e di pianificazione
* Ottime capacità di lavoro in team
* Conoscenza della lingua inglese, scritta e parlata
